If you have been ill for 7 days or less, you don’t need to see a doctor. You can complete a Self-Certification form yourself. You do not need to speak to a GP for this.

Self-Certification Form

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates.

If you have recently had an operation in a hospital your consultant/clinician should have issued you with a FIT note already. If not, please contact the hospital department to arrange and not the practice.

If you need a first FIT note after the 7-day self-certification period then you will need to make a GP appointment by contacting the surgery to discuss your illness.
